How to Use a Transition Survey in Special Education

Transition assessments are a require part of student IEP meetings in high school. They are a way for students and their teams to plan for their lives after high school. I’ll show you how to use a transition survey in special education!

What is a Transition Survey?

Transition assessments are a required to be included in IEP meetings when a student enters high school. This introduces students to transition planning and job exploration. This resource includes visuals to help students make individualized choices, mark interests, and share their wants and needs following graduation from special education programs. Shop the Transition Surveys!

Students can complete this assessment at the beginning of high school, and update at each IEP meetings, or this can be used just for students in the transition period between high school and graduation.

Using Visuals in Transition Surveys

In this resource, you will get three transition surveys with leveled supports. There are visuals, check boxes, and and multiple choice options. These transition surveys use visuals to help your students understand the various types of transitioning planning and job exploration. With real pictures, your students can make real world connections to their planning.

Transition planning can be overwhelming for students. They have been in school for nearly their entire life, so that is all they know. Expecting them to plan for their life after high school can be difficult if they aren’t sure what their interests are. Use this transition survey to review hobbies and interests your students may have, and have them check which ones they are interested in. This will help them make job planning and hobby decisions.
